Cleveland, Cincinnati organizations receive CMS Accountable Health Community grants

United Way of Greater Cleveland and the Health Collaborative in Cincinnati are among 32 organizations in the country to receive federal money to support efforts to link health care and social services (Source: “United Way of Greater Cleveland receives $4.5 million grant to address health-related social needs,” Crain’s Cleveland Business.

The Centers for Medicare and Medicaid Services awarded each organization a $4.51 million grant to establish the CMS Accountable Health Communities Model, which aims to address health-related social needs of Medicare and Medicaid beneficiaries, according to a news release.

" The Accountable Health Communities Model addresses a critical gap between clinical care and community services in the current health care delivery system by testing whether systematically identifying and addressing the health-related social needs of Medicare and Medicaid beneficiaries’ through screening, referral, and community navigation services will impact health care costs and reduce health care utilization," according to CMS’s website.
