HHS selects two Ohio sites for care transition pilot

The federal Department of Health and Human Services’ Partnership for Patients program announced today the first six sites for its community-base care transitions program, and two of the six are in Ohio.

According to an announcement from the Partnership for Patients, the Akron Canton Area Agency on Aging (A/C AAA) and the Southwest Ohio Care Transitions Collaborative were both selected for the pilot program created under the ACA to test ways to make transitions of care safer and smoother.

The A/C AAA is working in partnership with 10 acute care hospitals within its service area, according to the release. The Soutwest Ohio Care Transitions Collaborative serves the Cincinnati Metropolitan Statistical Area and surrounding counties in Kentucky, Indiana and Ohio.

The Partnership for Patients is a public-private partnership of hospitals, employers, physicians, nurses and patient advocates, along with state and federal officials, that was formed by HHS earlier this year to identify ways to improve the quality of hospital care.
