MetroHealth launches medical home pilot in Cleveland
MetroHealth System launched this week its "patient-centered medical home" pilot project (Source: “MetroHealth launches Partners In Care medical homes pilot,” MedCity News, March 2, 2009).
Called MetroHealth Partners in Care, the project aims to expand access to care and lower costs by organizing a coordinated team of medical professionals centered around patient needs, the Cleveland area’s health safety-net program announced.
Starting at its Lee-Harvard Health Center on the east side of Cleveland, a team of doctors, nurses, social workers, nutritionists, mental health professionals and financial counselors will coordinate care for qualified uninsured patients. The team is designed to provide a medical home for as many as 4,000 patients.
“We’ve got all the ingredients — the full continuum of care, the disease management programs, great primary care resources that we manage in a little bit different way that’s going to have much better outcomes for the patients,” said Mark Moran, MetroHealth’s interim CEO.
