OHT submits federal innovation grant proposal for dual-eligibles

Earlier this month the newly created Governor’s Office for Health Transformation submitted a proposal to the federal Center for Medicare and Medicaid Innovation for the creation of a demonstration model to integrate care for dual-eligible individuals (Source: “OHT's CMMI Proposal to Improve Care Coordination,” OHT Web site).

“The proposal is based on the common-sense idea that care coordination should serve the individual, not the system in which the individual is served--one person, one point of care coordination, regardless of payer, service or setting,” the proposal states.
