State Health Assessment (SHA) regional forums
In October 2018, the Health Policy Institute of Ohio hosted a series of regional community forums on behalf of the Ohio Department of Health. These forums provided stakeholders from a wide variety of sectors with an opportunity to identify health-related priorities, strengths, challenges and emerging trends for all areas of the state. The information gathered during these forums will inform the next State Health Assessment (SHA), State Health Improvement Plan (SHIP) and the Maternal and Child Health Block Grant and Maternal, Infant and Early Childhood Home Visiting Needs Assessments.

- 2017-2019 State Health Improvement Plan
- Ohio Department of Health (ODH) population health planning guidance and other SHA/SHIP materials posted on the ODH SHIP page
- Community health assessments and plans released by local health departments and hospitals in your county, and hospital community benefit information for hospitals in your county, posted on the ODH Population Health Plans and Assessments repository on the ODH SHIP page
- County Health Rankings data for your county
